Terry Manley - Pharmacy Technician of the Year
Congratulations to Terry Manley, Night Shift Pharmacy Technician in Central Pharmacy who was selected as the UK HealthCare Pharmacy Technician of the Year. Gayle Banks Retires
Congratulations and best wishes to Gayle Banks, CPhT on her retirement from UK Hospital. Gayle was a Pharmacy Technician who's career at UK spanned five decades with 23 years of cumulative service. Her commitment and expertise rendered most recently to Kentucky Children's Hospital patients has been greatly appreciated and her "Best Practices" will be missed. Her guidance and training for pharmacy technicians, residents and pharmacists has been instrumental in the careers of hundreds. Best wishes in retirement.

The Pharmacy Research and Education Committee acts as a central coordinating body for the development of effective pharmacy-coordinated investigational drug services, promotes practice-based outcomes research, supports programs aimed at the professional education and development of pharmacy personnel, and facilitates institutional relationships with Colleges of Pharmacy.

Rapp – UK Pharmacy Icon - Retires
Robert Rapp, Pharm.D., FCCP, retired after a forty year career at the University of Kentucky. Having held positions from Pharmacy Intern through College of Pharmacy Department Chair, Professor, and Associate Director of Pharmacy Services, Bob has demonstrated unparalleled leadership and innovation in many of the outstanding pharmacy programs at the hospital and the college. Bob has been a leader in his own practice, locally, regionally, nationally and internationally in the worlds of pharmacy practice, pharmacy education, pharmacy administration, neurosurgery pharmacy practice, total parenteral nutrition, sterile products, and infectious diseases. A winner of the ACCP Parker Medal and UK Parker Award, Bob’s impact has been great. The memories and experiences have been truly unique and his presence will be sorely missed by the fine team of pharmacy residents, practitioners and leaders which he has encouraged, motivated, activated, educated, debated, precepted, empowered and mentored.

Mynatt Wins James Rhodes Award
Ryan Mynatt, Pharm.D. (R#357), was awarded the James Rhodes Award for 2009 from the University of Kentucky College of Pharmacy. The award is for either a PY4 Pharmacy Student or PGY1 Resident at UK who was a UK College of Pharmacy graduate. The selection is based upon demonstration of a consistent commitment to clinical pharmacy practice. The candidate should be of outstanding moral character and viewed as consistently honest and trustworthy by peers. Congratulations Ryan!

Pharmacy Services Distributes Nutritional Products
Effective April 1, 2009, Pharmacy Services began responsibility for distributing nutritional products including adult and pediatric enteral tube feeding supplements and infant formulas. This major distribution transition from Food Services to Pharmacy Services was preceded by assuming responsibility for specialty infant formula preparation in mid-January. Pharmacy Services now includes enteral nutrition as well as the longstanding parenteral nutrition provision. This is truly a team effort of pharmacy technicians, pharmacists, clinical dieticians, nursing and medical staff.

Kentucky Pharmacist Recovery Network (PRN) Appointment
Congratulations to Jared Combs, Pharm.D. who was recently appointed by the Kentucky Board of Pharmacy to serve on the Pharmacist Recovery Network (PRN) Committee. The PRN Committee works with the Kentucky Board of Pharmacy and serves as a source of information for practitioners seeking help negotiating their way through the discovery of their disease, treatment, aftercare, and petition for reinstatement. Jared, now in recovery for 8 years, has also authored a book titled Incomprehensible Demoralization - An Addict Pharmacist's Journey to Recovery.

Emergency Department Pharmacist Team
Initiated in November 2006 with our first Emergency Medicine Clinical Pharmacist Specialist, David Santrock, Pharm.D., Pharmacy Services now completes the Emergency Department Pharmacist Team with the addition of Kyle Weant, Pharm.D., BCPS. The University of Kentucky HealthCare was the first in the Commonwealth of Kentucky to staff full-time pharmacists in the Emergency Department, another "Best Practice".

The Student and Employee Health Services Pharmacy satellite opened Monday in the new University Health Services building (830 South Limestone), room 129. This pharmacy serves the UK student population and UK employees for Workers Care related prescriptions and is open Monday through Friday 8AM to 5:30 PM. The pharmacy also has OTC and sundry items available for self selection.

UK Healthcare Pharmacy Intern Program
The creation of an organized Pharmacy Intern Program will serve many purposes for UK Pharmacy students and UK Healthcare patients. Goals include incorporating Pharmacy students practicing as Pharmacy Interns and extending clinical pharmacy services to a broader range of patients.  Pharmacy Intern Program Director, Lori Proeschel, Pharm D, BCPS, will be leading this program designed to introduce Pharmacy Interns to health-systems practice as well as serve as a recruitment tool targeted for UK pharmacy students as potential UK Healthcare pharmacy practitioners.

USP Chapter <797> Compliance Achieved
We are pleased to announce the completion of our Central Pharmacy Clean Room Renovation Project. Our existing central pharmacy IV admixture area has now been fully renovated into a separate ante room and a HEPA-filtered ISO Class 7 clean room. The clean room houses four ISO Class 5 laminar airflow workbenches. All non-immediate use compounded sterile products for adult patients in UK Chandler Hospital are prepared within the clean room. We applaud the adaptability and tenacity that has been displayed by our IV compounding technicians and pharmacists as we’ve worked through the phases of our renovation. Congratulations to the team involved!

Hamlin - Creativity Recognition
Congratulations to Ann Hamlin, RPh, Associate Director of Pharmacy Services for being nominated and receiving Honorable Mention for the A. Paul Nestor Creativity Award. Ann's nomination was submitted based upon creativity relative to financial purchasing of pharmaceuticals to reduce expenditures by over $100,000 per year. Ann's nomination is another evidence of a Best Practice in providing cost-effective medications to UKH patients.

Supply Chain Excellence

Thanks to a team of talented individuals from Pharmacy Services, Information Technology and Cardinal Health, a successful conversion to Pyxis® 3500 was made the week of January 7, 2008. This conversion upgrades UK Hospital’s Pyxis application. Pyxis® 3500 maintains the same user friendly touch screen features of the Pyxis® 3000 while also providing one of the most secure information technology environments currently available. Special thanks to our own UK Pharmacy Services Pyxis Administrator, John Baechler, CPhT for his instrumental role in this and every Pyxis related project implementation.

ASHP Foundation Award for Residency Excellence
Congratulations to all the residents, preceptors, staff and administration of the University of Kentucky (PGY1) Pharmacy Residency Programs for being awarded the 2007 Program Award for Excellence in Residency Training by the American Society of Health-System Pharmacists (ASHP) Foundation. The award will be presented at the ASHP Midyear Clinical Meeting in Las Vegas in December. eMAR Goes Live
After a successful eMAR pilot program on 5South and CCU, the electronic medication administration record (eMAR) went live for all adult patients this past week.  Kentucky Children's Hospital will complete inpatient implementation on November 6.  Electronic documentation of medication administration is now made in Sunrise™ Clinical Manager (SCM; Eclipsys Corp.).  Special thanks to Gary Coutts, R.Ph., Clinical System Coordinator, ICIS Projects as well as the support and effort of hundreds of nursing, pharmacy and IT personnel.

UK at PPAG in Virginia
UK Pharmacists/Residents (Kristi Southers, Pharm.D., Whitney Tucker (formerly Galloway) Pharm.D., Kim Deady Pharm.D., Zach Ramsey Pharm.D. and Robert Kuhn Pharm.D.) presented three abstracts at the Pediatric Pharmacy Advisory Group (PPAG) meeting in Portsmouth, VA in September.  In addition, Dr. Kuhn was presented the inaugural offering of the PPAG President’s Citation Award for his outstanding leadership and service to the PPAG.  Great expectations, great care, great outcomes!  Another Best Practice!

Pharmacy Services Goes to Camp
Several members of our pharmacy staff recently participated in Indian Summer Camp, Kentucky’s camp for kids with cancer. Over 80 children 6-17 years of age who have or have had cancer attended, along with nearly 50 volunteers. Nina Farhoudi, Pharm.D. coordinated the safe and efficient provision of pharmacy services. Also lending a hand were April Anderson, Pharm.D., Julia Ho, Pharm.D., Bojana Stevich, Pharm.D., Angie Bachtel, CPhT, and several UK pharmacy students.  Aaron Cook, Pharm.D. also spent his 8th year at camp as a counselor. Congratulations on a great job in leading the way to improving medication safety in a hectic environment and helping to give the campers a break from their illness and let them just be kids for a while!

Congratulations to Kimberley Hite, Pharm.D., MS for being awarded the UK Nursing Appreciative Inquiry Award for 2007. Evans Selected Commander
Tim Evans, Pharm.D. Clinical Staff Pharmacist, Central Pharmacy, UK HealthCare and Lieutenant Colonel, U.S. Army Reserve has been selected Commander of the 7241st Medical Support Unit. This Reserve medical unit is comprised of medical specialists whose mission is to support Army Medical Centers and provide trained medical personnel to deploying units. Piecoro - One of the World's First Clinical Pharmacists Retires from UK
John Piecoro, Pharm.D. will be retiring from the University of Kentucky College of Pharmacy in June 2007. John has had a 38 year career with the University of Kentucky and served as one of the world's first clinical pharmacists at the University of Kentucky Hospital, practicing in Pediatrics. John has been serving as Professor at the University of Kentucky College of Pharmacy and is retired as a Colonel for the Kentucky Army National Guard after more than 28 years of service. Congratulations to April Miller, Pharm.D. who has been selected the UK HealthCare Resident of the Month for March 2007. Approximately 500 House Staff members are eligible for this award. WORx AC® Go Live Complete
The Kentucky Clinic Pharmacy has implemented the WORx AC® outpatient pharmacy prescription software. Thanks to Kentucky Clinic Pharmacy pharmacists, technicians, patient accounts and MCIS staff for their contributions in this process. Over 1,000 prescriptions orders were filled on day one, and although lines were a bit longer than usual, our patients will gain from this pharmacy software with enhanced functionality, efficiency and security.

Pharmacy Administration Relocated
After over thirty years in the C building of the hospital, Pharmacy Administration and related offices have been relocated to H110 (and nearby offices). The new location houses staff for pharmacy administration and medication safety as well as offices for clinical pharmacist specialists in cardiology, medication use policy, infectious disease, emergency medicine, internal medicine/therapeutic drug monitoring; the chief pharmacy resident and pediatric pharmacy residents.

Markey Outpatient Infusion Pharmacy Opens - Second Floor
After only three years on the first floor of the Whitney Hendrickson Building, the increasing demands for outpatient oncology pharmacy services have necessitated the construction of the second floor outpatient infusion pharmacy satellite.  The satellite opened on August 14th and construction is near complete to allow for doubling of patient care capacity for this Markey Cancer Center service.

Clifford E. Hynniman, MS, RPh will be retiring from the University of Kentucky College of Pharmacy in June 2006. Long Standing Practice
Congratulations to Pharmacy Technician Robert King who recently celebrated his 40th anniversary at the University of Kentucky Hospital.  Robert is one of few who have practiced under all four pharmacy directors at UKH.  Although we know that Paul Parker "invented" unit dose, we all know who was involved with implementing it.  Robert was awarded Technician of the Year in 2003 and is still demonstrating the "Best Practice" in the oral specials, compounding and prepackaging areas.  Congratulations for your long standing service Robert!

Medication Reconciliation at UK Hospital
Effective January 18, 2006 a new procedure will be established for conducting interdisciplinary medication reconciliation in the patient medical record for University of Kentucky Hospital inpatients. Physicians should no longer document the medication history in the History and Physical form in the patient medical record. Nurses should no longer document the medication history in the Nursing Admission Assessment form. All disciplines will document the medication history in the Medication Reconciliation Database form which will be included in all inpatient patient medical records. The process of reconciliation includes verification of all current medications being taken, clarification that the medication and dosing is appropriate, ongoing or intentionally discontinued, and reconciled to document changes in regimens.

Craig Martin Named Clinician of the Year by National Organization
The Society of Infectious Diseases Pharmacists (SIDP), an association of health professionals dedicated to promoting the appropriate use of antimicrobial agents, has presented Craig Martin, Pharm.D., with the Clinician of the Year Award for his work in antimicrobial management at UK Hospital. The award was presented Dec. 20 at the annual SIDP meeting in Washington D.C.  Martin is Assistant Professor at the UK College of Pharmacy and Infectious Diseases Clinical Specialist at the UK Chandler Medical Center.

Armitstead Named the 2005 Clifton J. Latiolais Award Recipient
Today at the ASHP Midyear Meeting in Las Vegas, John Armitstead, MS, RPh, Director of Pharmacy Services, received the Clifton J. Latiolais Award and presented to alumni, residents and friends of The Ohio State University Masters/Residency Program. The award is presented to distinguished graduates of the Ohio State program for their contributions and dedication to pharmacy practice that was both characteristic and important to Dr. Latiolais, the Director of Pharmacy at OSU Hospitals from 1958-1983.  Congratulations to Mr. Armitstead for receiving this significant and rewarding achievement.

Yam Recognized with ASHP Foundation Pharmacy Resident Health Services Grant Program
Congratulations to Felix Yam, Pharm.D. for having his residency project selected as one of the winners of the ASHP Foundation Pharmacy Resident Health Services Grant Program. Teamed with faculty member Mikael Jones (R#299), Dr. Yam's project is entitled, "Clinical Inertia: Time to Intervention of Critical A1C Values in Two Primary Care Models for Patients with Type 2 Diabetes: PharmacistCARE vs. Usual Care". The University of Kentucky Chandler Medical Center Pharmacy Services has been awarded the Kentucky Society of Health-System Pharmacists (KSHP) Institution of the Year Award at the KSHP Fall Meeting. Activities cited in making this determination were: Computer Prescriber Order Entry (CPOE) support and implementation, Antimicrobial Management Team activity and outcomes, ScriptPro automation implementation, Pyxis Cubie and Ordered Medications Not Loaded (OMNL) program implementation, and Counter Terrorism Training throughout the Commonwealth. The KSHP Institution of the Year acknowledges the institution that develops and implements state of the art processes to improve patient care and contributes to the practice of pharmacy. Armitstead Installed as President of KSHP
John Armitstead, MS, RPh, Director of Pharmacy Services and Assistant Dean of Medical Center Pharmacy Services has been installed as President of the Kentucky Society of Health-System Pharmacists (KSHP) for 2005-2006. John's inaugural speech focused on the 2015 Vision and the need to move from Your Patients, Your Practice, Your Health-System and Your Profession to Our Patients, Our Practices, Our Health-Systems and Our Profession. The basis of this vision is the ASHP 2015 Initiative.

Richard Griffieth Named Employee of the Month
Congratulations to Richard Griffieth, R.Ph., for being selected as the UK Hospital Employee of the Month for August 2005.  Richard was selected for the outstanding service he provides to patients of the University of Kentucky Hospital through his long term service on the night shift.  Excellent service continues to be provided 24/7 thanks to Richard and the night shift pharmacy team. Faculty Member Leading Residency Training Nationally
Congratulations to Kelly M. Smith, Pharm.D., Associate Professor of Pharmacy, Director of the Drug Information Center, and Pharmacy Practice Residency Program Director at the University of Kentucky for her appointment as Chair of the American Society of Health-System Pharmacists Commission on Credentialing (2005-2006). The Commission on Credentialing is responsible for accreditation of more than 800 pharmacy residency and technician training programs throughout the United States.

Kentucky Clinic Pharmacy Awarded KADAP Contract
On July 1, 2005 the Kentucky Clinic Pharmacy began exclusive service to a state-wide network through the Kentucky AIDS Drug Assistance Program (KADAP).  KADAP is a Ryan White Grant program administered by the HIV/AIDS Branch of the Kentucky Department for Public Health that provides medications to HIV positive individuals who could not otherwise afford them.  The service consists of preparation and dispensing of KADAP formulary medications, counseling and education services, compliance counseling and tracking and mail order service for delivery when requested.

Freml named as the Rufus A. Lyman Award Recipient
Today at the American Association of Colleges of Pharmacy (AACP) Annual Meeting in Cincinnati, Jared Freml, Pharm.D., Oncology Pharmacy Practice Resident, and his colleagues from the University of Iowa were honored as the 2005 recipients of the Rufus A. Lyman Award.  This award is selected annually by the AACP Board of Directors to recognize the most notable original research or scholarly work published in the American Journal of Pharmaceutical Education in the preceding year.

CPOE Implemented in Kentucky Children's Hospital and PACU
On June 15, 2005, the UK Healthcare team successfully implemented our Computerized Provider Order Entry system (CPOE) in the Kentucky Children's Hospital and in the Post Anesthesia Care Unit (PACU), completing inpatient implementation of CPOE. Pager Text Messaging Tool Expanded to Include Unit Liaisons
We've expanded our Pager Text Messaging Tool to include the Unit Liaisons as well as the pharmacists.  By using this tool available on the Contact Us page, you can now send a text message for missing medications, pyxis issues and deliveries.  Of course you can continue to utilize the current pagers by phone if would like to speak with the Pharmacist or Unit Liaison for your area, but we believe this new system will make your simple communications more efficient.

Nutrition Support Service Now Online
We're pleased to present another new section of our website. Training/Assessment Program for Compounding of Sterile Preparations Online
This program, recently presented by Philip Trapskin, Pharm.D. at the Spring Meeting of the Kentucky Society of Health-System Pharmacists presents the USP 797 standards related to training and assessment.  Based on the successful program implemented at UK Hospital, it provides practical approaches for the development and implementation of programs at other institutions through the files made available online.  View the program in the new Personnel Training section of the Pharmacy Resources accessible via the menu on the left.

Exacta-Mix™ 2400 Compounder Implemented
The Exacta-Mix 2400™ Compounder (Baxa Corp.) is an automated system that delivers both micro and macro ingredients accurately for compounding of Total Parenteral Nutrition (TPN) solutions.  It handles 24 ingredients with precision through easy to use touch-screen technology and accurately delivers volumes as small as 0.2 mL. The EM2400 provides unprecedented patient safety through ingredient verification and formula selection and is now being used to prepare TPNs for the neonatal, pediatric, and adult patient populations at UK Hospital.

WORx Upgraded and SCM-WORx Interface Established
Pharmacy Services completed a major upgrade to the pharmacy order entry system (WORx; Mediware Inc.) and achieved a milestone in Integrated Clinical Information System (ICIS) project through the activation of the unidirectional interface between these systems.  Most new pharmacy orders in the Sunrise™ Clinical Manager client are now electronically transferred for pharmacist review!  Thanks to pharmacists, technicians, Mediware (WORx) staff, ICIS staff, MCIS staff, Pyxis staff and nursing staff for all their contributions in this process.

The UK Healthcare team is proud to announce the implementation of our Computerized Provider Order Entry system (CPOE) on an additional floors and services.  The Sunrise™ Clinical Manager client (SCM; Eclipsys Corp.) went live on October 26th on the 6th floor, in the CCU, and in the CTICU including patients on the Cardiology, Cardiothoracic Surgery, and Family Medicine services.

Antimicrobial Management Program Wins National Best Practice Award
The Antimicrobial Management Team, a collaboration of the Infectious Diseases Division and Pharmacy Services, received the 2004 Best Practices Award in Health-System Pharmacy for their submission, Improving Antimicrobial Use at a University Hospital: Results of the First Five Years of a Multidisciplinary Antimicrobial Management Program.  The award recognizes "outstanding practitioners who have successfully implemented systems to improve patient care and demonstrate best practices".  It will be presented at the Opening Session of the ASHP Midyear Clinical Meeting in Orlando.  Congratulations Dr. Craig Martin, Dr. Ardis Hoven, and all those involved in this program!

The UK Healthcare team is proud to announce the arrival of our Computerized Provider Order Entry system (CPOE).  The Sunrise™ Clinical Manager client (SCM; Eclipsys Corp.) went live on July 21st on 3 North, Mother/Baby, Labor/Delivery, and 22 ancillaries.  Although not currently interfacing directly with the pharmacy order entry system (WORx; Mediware Inc.) this feature is a planned part of the Integrated Clinical Information System (ICIS) project.
