Therapeutic Use Criteria:

• Infants with BPD requiring medical management in the previous 6 months and <2 years of age at onset of RSV season

• Premature infants without BPD, but:

• Infants at highest risk: all infants born at <28 weeks gestation and <1 year of age at onset of RSV season

• Infants born at 29-32 weeks gestation and <6 months old at onset of RSV season

• Infants born at 33-35 weeks gestation, on a case-by-case basis, if other risk factors present:

- Multiple births
- Crowded household (>4 people or siblings)
- Child in daycare
- Smokers present in the home
