Potassium Infusion Guidelines - Children's Hospital

Potassium chloride intravenous boluses (K+ Runs) are NOT permitted for general use in the Children’s Hospital.  Exceptions include:
  • Pediatric intensive care unit; (0.25mEq/kg - 0.5mEq/kg)
  • Neonatal intensive care unit; (0.25mEq/kg - 0.5mEq/kg)
  • Hematology/oncology service, with order written by senior resident or attending physicians; (0.25mEq/kg).

Low serum potassium levels may be replaced by:

  • Increasing the potassium concentration in maintenance fluids for a limited number of hours to provide the extra potassium needed.
  • Providing oral replacement is also an option.

Note: For potassium runs in 4PIC/4NIC, dose = 0.25mEq/kg - 0.5 mEq/kg, maximum of 10 mEq/hour and not to exceed 1 mEq/kg/hr). For the H/O patients,potassium runs dose = 0.25mEq/kg.
