Orders for Augmentin® 500 mg tid are automatically converted to 875 mg bid. Documentation of this Pharmacy and Therapeutics (P&T) Committee-approved conversion can be found in the notes field of the patient’s medication administration record (MAR). This conversion does not apply to pediatric patients, who should be receiving doses based on patient body weight.
