The online
formulary is subject to continuing revision in the light of new therapeutic
knowledge. Medications are therefore added from time to time while others
may be deleted. These changes are approved by the Pharmacy and
Therapeutics Committee and are posted to the online formulary database after
each meeting. Details of the specific changes are noted in monthly update pdf ( )documents
and can be viewed from anywhere within the Medical Center. (
